Free Medical Transportation
Ordering Healthcare and Family Service Medical Transportation
•
 First you must have a proper Medicaid Identification number or be a parent, guardian, relative, or social worker representing the recipient.
•
 Once you have made a medical appointment, and can document it, you need to contact First Transit Inc . at 877-725-0569 to obtain a Reference Number and let them know you want to ride with
Alef Cab Co. 866-923-0900.
•
 With your reference number, and doctors or medical facilities information, it is important to immediately contact Alef Cab Co ., supply the necessary information (phone, address, date of birth etc.), and schedule your Free cab ride at least 2 days prior to your appointment.
•
 Return rides should be arranged before leaving your appointment, we recommend calling toll free, 866-923-0900 , a half hour before the appointment is over.
•
 Standing orders, more than one round trip, follow a little different procedure:
 
  1. Your doctor fills out a standing order form.
  2. Copies are faxed to both First Transit and Alef Cab Co.
  3. Processing by First Transit will take from one to two weeks; we therefore recommend placing your order in advance.
  4. Know your expiration date, so renewal won't cause unnecessary delays.
